This could be the Jan van Eyck, Dresden Triptych, 33 x 14, 33 x 28, 33 x 14, Dresden Gemäldegalerie AM-799-PS01 (CR brand), exchanged with Endymion Porter for a work by Palma Giovane, described in van der Doort WS 59, no 95. The issue is that this entry is described as a gift from Inigo Jones whereas in the exchange it is described as a Mantua piece, which is where the Dresden van Eyck is believed to come from

Very little, old, defaced, Altar Piece split in three pieces missing hangings, our lady in the middle, and her drapery defaced and spilt upon (two shutting doors, on one side St Catherine, on the other St Michael Archangel)
